How to Fill Out Vietnam's Health Declaration Form (July 2026)

Vietnam has reintroduced its health declaration form for arriving travellers from 1 July 2026. If you're planning a trip, or bringing colleagues, clients, or family over, the form itself is straightforward once you know what each field is asking. The only real hurdle is that it's written in Vietnamese.
This guide walks through every field you're likely to see, with a plain-English translation, so you can fill it in at the airport or border without second-guessing.

Part 1: Personal information
- Họ tên (viết chữ in hoa): Full name, in CAPITAL letters, exactly as printed in your passport.
- Năm sinh: Year of birth, four digits (e.g. 1990).
- Giới tính: Gender. Nam = Male, Nữ = Female.
- Quốc tịch: Nationality, exactly as shown in your passport.
- Số định danh cá nhân / CC / CCCD / Hộ chiếu: ID or passport number. For foreign visitors, this is your passport number (Hộ chiếu = Passport).
Part 2: Travel and itinerary
- Thông tin đi lại: How you're arriving. Tàu bay = Plane, Tàu thuyền = Boat, Phương tiện đường bộ = Land transport, Khác = Other.
- Số hiệu phương tiện: Flight or vehicle number, e.g. VN570.
- Số ghế: Seat number. Fill in if you have one, leave blank if not.
- Ngày khởi hành: Departure date, format dd/mm/yyyy.
- Ngày nhập cảnh: Date you enter Vietnam, format dd/mm/yyyy.
- Địa điểm khởi hành (tỉnh/quốc gia): Where you're travelling from (city / country).
- Địa điểm nơi đến (tỉnh/quốc gia): Your destination in Vietnam (e.g. Ho Chi Minh City, Hanoi, Da Nang).
- Trong vòng 14 ngày qua, Anh/Chị có đến quốc gia/vùng lãnh thổ nào không?: Any countries or territories you've visited in the past 14 days. List them if so.
Part 3: Contact details in Vietnam
- Địa chỉ nơi ở tại Việt Nam: Your address in Vietnam: hotel name plus the full address.
- Điện thoại: Contact phone number. A local SIM or a roaming-enabled mobile works best.
- Email: Email address.
Part 4: Health in the past 14 days
Mark each symptom Có (Yes) or Không (No):
- Sốt: Fever
- Ho: Cough
- Khó thở: Difficulty breathing
- Đau họng: Sore throat
- Nôn / buồn nôn: Vomiting / nausea
- Tiêu chảy: Diarrhoea
- Xuất huyết ngoài da: Skin bleeding (petechiae)
- Nổi ban ngoài da: Skin rash
Part 5: Vaccinations and exposure
- Liệt kê tên vắc xin hoặc sinh phẩm y tế đã sử dụng: List any vaccines or medical products you've received.
- Đến trang trại chăn nuôi / chợ buôn bán động vật sống / cơ sở giết mổ động vật / tiếp xúc động vật: Have you visited a livestock farm, live-animal market, or slaughterhouse, or had contact with animals? Có = Yes, Không = No.
- Trực tiếp chăm sóc người bệnh truyền nhiễm: Have you directly cared for anyone with an infectious disease? Có = Yes, Không = No.
Part 6: Declaration and signature
- Tôi cam kết những thông tin trên là đúng sự thật…: A declaration that the information you've given is true, and that you understand the consequences of providing false details.
- Ngày tháng năm 20…: The date you complete the form, dd/mm/yyyy.
- Ký tên: Signature. Sign by hand on paper forms; for online submission, follow the e-signature instructions.
A few things that make entry smoother
- If a form is handed out on the plane, fill it in before you land.
- Keep your passport and accommodation details within easy reach.
- Dates use day/month/year, which is easy to transpose if you're used to month/day.
- Answer the health and travel questions honestly. Giving false information on the declaration can carry penalties.
